Put simply, a “superbill” is just a billing document that you submit to your insurance for reimbursement. It will contain a lot of information so that your claim can be processed, including names, contact info, my professional qualifications, a diagnosis code, a code for the type of service(s) provided, and the amount you paid.
Importantly, many insurance providers want the superbill attached to a specific form they create (which asks for even more information!). It’s best to call them and ask for the form they are currently using. Remember, you can always speak with a representative from your insurance provider by calling the 800 number on the back of the card.
